We have been around since 2001 and are focused on affecting change that will help people have better lives, become better off and help people make better sense of the world around them, including navigating and making change to the systems that do not work for many people in Cornwall.
 
We’re now on the lookout for a number Social Prescribing Link Workers to work across Kerrier delivering the Social Prescribing service, with some roles focusing on working in conjunction with schools, surgeries and other agencies that work with young people aged 11 to 25, and others a more general social prescribing role.
 
This is a fixed-term contract until the 31st March 2022 (which has the potential to extend subject to funding).

You’ll be at the heart of the action, working in a front-line position and ensuring that the people we work with are enabled to grow, develop and flourish within their communities.
 
Your Role
 
As a Social Prescribing Link Worker, you will be tasked with guiding and supporting people in the Kerrier area, on their journey to wellness, social inclusion, learning or work.
 
Some of these roles will focus on those aged between 11 and 25 and their families, where you will manage a caseload that has been referred to you by health professionals and will help them to overcome barriers to achieving their goals.
 
You will support programme participants to take positive action, using an individual-centred approach and taking what matters to them into consideration.
 
Your role will also involve:
 
- Working directly with participants in both group and 1-2-1 settings
- Providing advice and guidance in an inclusive way
- Liaising with referring agencies to obtain appropriate and relevant information
- Building relationships with local specialist project partners
- Working with safeguarding teams at GP surgeries, schools, CN4C and other relevant agencies
 
About You
 
To be considered as a Social Prescribing Link Worker, you will need:
 
- At least one year’s experience in a similar role, working in a people-centred environment
- An understanding of how to overcome the barriers for people from disadvantaged communities and for people facing multiple and complex needs
- At least one year’s experience of managing a caseload
- Experience of working with young people aged 11-25 and their families, where appropriate
- At a minimum, level 2 English and Maths qualifications, or an ability to demonstrate literacy and numeracy and a willingness to work towards a Level 2 qualification
- A Level 3 qualification or above in Information, Advice and Guidance, Mentoring or Coaching, or equivalent experience for the roles working with young people and families
