Cornwall Neighbourhoods for Change (CN4C) is a charity and a social enterprise, which aims to transform the quality of life for people, places and communities.

We have been around since 2001 and are focused on affecting change that will help people have better lives, become better off and help people make better sense of the world around them, including navigating and making change to the systems that do not work for many people in Cornwall.

We’re now looking for a Social Prescribing Link Worker to join us on a full-time, fixed-term contract until the 31st March 2022 (which has the potential to extend subject to funding).

Your Role

As a Social Prescribing Link Worker, you will be tasked with guiding and supporting young people in Cornwall on their journey to wellness, social inclusion, learning or work.

With a focus on those aged between 11 and 25 and their families, you will manage a caseload that has been referred to you by health professionals and will help them to overcome barriers to achieving their goals.

You will support programme participants to take positive action, using an individual-centred approach and taking what matters to them into consideration.

Your role will also involve:

- Working directly with participants in both group and 1-2-1 settings
- Providing advice and guidance in an inclusive way
- Liaising with referring agencies to obtain appropriate and relevant information
- Building relationships with local specialist project partners
- Working with safeguarding teams at GP surgeries, schools, CN4C and other relevant agencies

About You

To be considered as a Social Prescribing Link Worker, you will need:

- At least one year’s experience in a similar role, working in a people-centred environment
- Experience of working with young people aged 11-25 and their families
- An understanding of how to overcome the barriers for people from disadvantaged communities and for people facing multiple and complex needs
- The ability to manage a caseload
- An understanding of working with people through traumatic events
- Good IT skills and proficiency in MS Office
- At a minimum, Level 2 English and Maths qualifications, or the ability to demonstrate literacy and numeracy and a willingness to work towards a Level 2 qualification
- A Level 2 qualification or above in Information, Advice and Guidance, Mentoring or Coaching, or equivalent experience
- Flexibility to work evenings and weekends

A track record of working in partnership with voluntary and statutory organisations would be beneficial to your application, as would a safeguarding qualification at Level 2 or above.

Other organisations may call this role Outreach Worker, Social Worker, Health and Social Worker, Social Prescribing Advisor, Community Advisor, Community Support Worker, or Support Worker.
