Who we are:

Westcountry Case Management is an established service, providing complex case management to client of all ages living with a variety of conditions, including birth injuries, acquired brain injuries, spinal cord injuries and multiple diagnoses. We work with clients undergoing personal injury or negligence claims, and our case managers need to be able to communicate effectively within that process.

We are seeking dynamic, passionate and kind individuals with the following:

• Professional health or social qualification, with 7rs experience preferred, such as occupational therapist, social worker, nurse, physiotherapist, speech and language therapist or clinical psychologist.
• Ability to work with the WCM values of integrity, sustainability, compassion, collaboration, quality and professionalism
• Full UK driving license with the ability to travel to and work in client sown homes
• Robust clinical reasoning skills to underpin and justify decisions made
• Excellent record keeping and report writing skills
• Highly effective verbal and written communication skills
• Ability to build therapeutic and professional relationships quickly
• Ability to work autonomously and also as part of a team, taking responsibility for your own actions
• Ability to demonstrate excellent time management and co-ordinate others when working to deadlines
• Ability to apply new learning to novel situations
• Must be IT literate and have the ability to learn new systems quickly
• Strong problem-solving skills and the ability to manage difficult and at times, volatile situations

Our Case Managers work with our clients and their families to assess rehabilitation needs, report our recommendations, commission services and lead multi-disciplinary teams, providing rehabilitation and support in the community to assist clients to reach their optimum potential.

Westcountry Case Management are registered with the CQC and are rated outstanding in all 5 areas. We are also registered with the CIW for our work in Wales. We have a strong clinical leadership team and robust induction and ongoing support for case managers.

We cover the whole of the South and West of England and Wales and are particularly seeking experienced or potential case managers located in south Devon, Somerset, Wiltshire, Bristol or surrounding areas.

Salary and benefits:

• Salary of £39-45,000.00 dependent on experience
• Mileage allowance
• Company pension scheme
• Bonus scheme
• Full training and supervision
• 5 weeks annual leave, plus bank holidays and birthday off
• 2 weeks full sick pay and 2 weeks half pay
We are also happy to discuss case managers wishing to work on a self-employed consultancy basis with us
