Job Type: Permanent Full Time

Location: Balham & other London locations
Shift Pattern: Generally Monday to Friday 9am till 5pm; though with some evening and weekend work is to be expected.

About the role

The role of a Shared Lives Co-ordinator is to support and monitor approved Shared Lives Carer Providers supporting vulnerable adults in the Care Providers own home - somewhat similar to fostering. The Shared Lives Certitude Scheme operates across Lewisham, Lambeth, Richmond, Wandsworth and Hounslow. The role will involve visiting Carer Providers in their home and the role will involve elements of lone and home working. You will receive a comprehensive induction and support from the Shared Lives Manager and the wider Shared Lives Certitude team.

Duties will include:

- You will manage a caseload of carers who support people with learning disabilities in their own home.
- You will offer on-going support to the carers through coaching mentoring and advocating on their behalf.
- You will lead on a number of initiatives including training packages, support groups and new assessments.
- Working as part of a small team you will understand what it takes to deliver an outstanding service to the people we support and their carers based on person centred thinking and approaches.

About you

To be a Shared Lives Co-ordinator at Certitude, the following are essential:

- Track record of enabling choice and control to be a reality for people with learning disabilities.
- Track record of supporting individuals to become more independent or maintain life skills.
- Experience of supporting people to positively engage as citizens within their community, cultivate relationships and being an active member of the local community.
- Excellent administration, IT and communication skills
