Overview

The Home from Hospital Service helps people reconnect with their community or by engaging in activities of interest. The Service aims to reduce recovery time following a hospital stay, reduce readmissions as well as the overall impact of isolation and loneliness on wellbeing.

The Role

As a Service Co-ordinator, you will provide a Home from Hospital support service to recently discharged patients in the Shrewsbury area. Upon leaving the hospital, you will be responsible for transporting patients to their homes and continuing to assist them with their daily needs for up to six weeks.

We’re currently looking for a Service Co-ordinator to join us on a fixed-term contract until 30th November 2022. Extension of the contract is possible if further funding is obtained.

In this role, you will be responsible for:

- Transport around the local area (using your own vehicle - all mileage fully reimbursed on a weekly/monthly basis)
- Shopping and prescription collection/delivery
- Hydration and nutrition advice and support
- Physical exercise

You will be responsible for weekly visits to clients, ensuring they receive personalised support to meet their own identified goals and targets. You will not be responsible for managing medication or personal care.

Additionally, you will provide support to the Service Manager in the recruitment and training processes of new volunteers.

Working Hours

We are looking for someone to join us working 25 hours per week however we are also happy to discuss individual working days and working hours to suit your situation.

This could include weekends and bank holidays and there may also be opportunities for paid overtime in busy periods or to cover absence.

What you’ll need

- A full, valid driving licence and access to your own vehicle (mileage reimbursed at government rate)
- Relevant experience of working with volunteers and supporting and motivating them on a daily basis
- Knowledge and understanding of the NHS, Health and Social Care Partnerships and Local Authority commissioning including Health and Social Care bodies would be an advantage
- Excellent communications ability at all levels
- Proficiency in standard IT systems
- Ability to act on own initiative within agreed parameters
